CERTAIN critical train standard operating procedure (SOP) were overlooked, leading to the May 24 Light Rail Transit (LRT) crash involving two trains and injuring 64 passengers, said Transport Minister Wee Ka Siong.

Wee said the error led to the collision between an empty train and the one with passengers.
